Illinois state mandates that your child have a physical for the Kindergarten, 6th grade, and 9th grade years.  This must be completed before registration, or your child will not be allowed to start school until it is completed.  Please don't wait to make your appointment because the physicians fill their openings quickly.

• IL Certificate of Child Health Examination (School Physical) - This form can also be used as a sports physical.
  (Required for entry into Kindergarten, 6th and 9th grade. Also required for any new student enrolling in an Illinois school for the first time.) A Sports Physical must be completed before participation in a sports activity.

• Pre-participation Examination IHSA/IESA (Sports Physical)

• Proof of School Dental Examination Form - Required for Kindergarten, 2nd, 6th and 9th grades.

• State of Illinois Eye Examination Report - Required for Kindergarten and any new student enrolling in an Illinois school for the first time.

• School Medication Authorization Form - Required to be completed by a physician if the student is to keep medication at school.

• Authorization for self-carry medication.
  (Required to be completed by parent and physician if the student carries an inhaler or epi-pen at school.)

• Diabetes Medical Management Plan (DMMP)

• Seizure Action Plan (SAP)

• Food Allergy Action Plan - Emergency Care Plan

• Family Food Allergy Health History Form - ALLERGY ASSESSMENT

• IDPH - Asthma Action Plan

• Illinois Certificate of Religious Exemption to Required Immunizations and/or Examination Form

• Wabash County Health Department Vaccination Consent Form
